Clinical Assessment of Intracranial Tumors Undergoing Surgical Management in the Elderly

Abstract

|
Objective: The incidence of brain tumor in the older population has increased progressively. However, it may be controversial whether aggressive management of brain tumor in the elderly gives better outcome. The purpose of this study is to investigate clinical characteristics in older patients with brain tumor who have undertaken surgical management.
Materials and Methods: Sixty-nine patients with brain tumor who aged 60 years or older and have undertaken the surgical treatment were included. We reviewed medical records to analyze the demographic data, clinical presentations, histopathology, therapeutic modalities, and survival outcome in the elderly with brain tumor retrospectively.
Results: The most common clinical presentations were headache(43.4%), followed by memory loss(31.9%) and mental confusion(21.7%). The elapsed time from the onset of memory loss to diagnosis was longer(median duration£º47 days) comparable to other symptoms(p<0.05). The incidence of histopathology was meningioma(41.4%), metastatic tumor
(30.6%), glioma(14.7%), pituitary adenoma(5.3%), schwannoma(4.6%), and primary lymphoma(2.5%) in orders. Postoperative adjuvant therapy was performed in 45(65.2%) patients. Survival time was significantly reduced in patient aged 60 years or older. Major causes of mortality were postoperative systemic diseases which were the most common in pulmonary complications.
Conclusion: This study suggest that the older population with brain tumor commonly present headache and memory loss, and surgical resection of brain tumor in the advanced age should be decided by consideration of the medical conditions.
